Copy it to you mods folder (Default location: C:\Users\YOUR USER NAME\Euro Truck Simulator 2\Mods) Then edit your profile and enable the mod, apply and check out the paint shop for it.

I had luck using the paint changing paints again exiting paint shop going back in and doing it again.
